One of MAPS’ goals is to develop a statewide model for a “Culture of Learning, Justice and Accountability,” which is a culture that:

- strikes a balance between a punitive environment and a ‘blame-free’ culture;
- differentiates between individual behaviors and system failures; and
- recognizes humans make errors, yet should be held accountable for at-risk and reckless behaviors.

MAPS developed the “Statement of Support for a Statewide Culture of Learning, Justice and Accountability.” View this document, along with recommended tools and resources for achieving a culture of learning, justice and accountability in your facility. more

  National studies have found that more than 90 million Americans are unable to adequately understand basic health information. There is no more critical of a time to understand information, or have a high level of health literacy than before surgery or an invasive procedure.

Informed consent is an interactive process between a patient and provider that occurs before surgery or an invasive procedure.

MAPS created an informed-consent form designed to be used at Minnesota health-care organizations statewide. The MAPS form is written at a fourth-grade reading level that a majority of patients can understand. more
