One of MAPS’ goals is to develop a statewide model for a “Culture of Learning, Justice and Accountability,” which is a culture that:

- strikes a balance between a punitive environment and a ‘blame-free’ culture;
- differentiates between individual behaviors and system failures; and
- recognizes humans make errors, yet should be held accountable for at-risk and reckless behaviors.

  National studies have found that more than 90 million Americans are unable to adequately understand basic health information. There is no more critical of a time to understand information, or have a high level of health literacy than before surgery or an invasive procedure.

Informed consent is an interactive process between a patient and provider that occurs before surgery or an invasive procedure.

MAPS created an informed-consent form designed to be used at Minnesota health-care organizations statewide. The MAPS form is written at a fourth-grade reading level that a majority of patients can understand. more
